Priority is being given to NEW employees on staff who have been on the job less than 2 years. Priority will be given to NEW IAs, MUI Contacts, County Board, Agency Provider Managers, Admins, Supervisors, Nursing Professionals, and Individual Providers in meeting the requirements of the rule.

WHEN

HOW

Registration forms will be accepted via Email or FAX only. Each person interested in attending will need to submit a completed registration form. A confirmation will be sent within 3-5 business days after the registration form is received. If you do not receive a confirmation, you are not registered and will need to contact the MUI Unit at (614) 995-3810.
